RESUMO

The introduction of MRI was supposed to be a qualitative leap for the evaluation of Sacroiliac Joint (SIJ) in patients with Axial Spondyloarthropathies (AS). In fact, MRI findings such as bone marrow edema around the SIJ has been incorporated into the Assessment in SpondyloArthritis International Society (ASAS criteria). However, in the era of functional imaging, a qualitative approach to SIJ by means of conventional MRI seems insufficient. Advanced MRI sequences, which have successfully been applied in other anatomical areas, are demonstrating their potential utility for a more precise assessment of SIJ. Dixon sequences, T2-mapping, Diffusion Weighted Imaging or DCE-MRI can be properly acquired in the SIJ with promising and robust results. The main advantage of these sequences resides in their capability to provide quantifiable parameters that can be used for diagnosis of AS, surveillance or treatment follow-up. Further studies are needed to determine if these parameters can also be integrated into ASAS criteria for reaching a more precise classification of AS based not only on visual assessment of SIJ but also on measurable data.

RESUMO

Gaucher disease is reckoned for extreme phenotypic diversity that does not show consistent genotype/phenotype correlations. In Argentina, a national collaborative group, Grupo Argentino de Diagnóstico y Tratamiento de la Enfermedad de Gaucher, GADTEG, have delineated uniformly severe type 1 Gaucher disease manifestations presenting in childhood with large burden of irreversible skeletal disease. Here using Long-Read Single Molecule Real-Time (SMRT) Sequencing of GBA1 locus, we show that RecNciI allele is highly prevalent and associates with severe skeletal manifestations in childhood.

BACKGROUND: Magnetic resonance imaging (MRI) is a fundamental diagnostic tool in axial spondyloarthritis (SpA), allowing us an earlier diagnosis of the disease compared to radiography. OBJECTIVE: To compare the performance of a recognition test on SpA MRI lesions and theoretical knowledge, before and after carrying out an educational intervention (hands-on workshop). We also evaluated whether the successes in the tests were associated with the individual characteristics of the participants. METHODS: A test was carried out involving 10 questions (seven for image recognition and three for theoretical knowledge) before and after the attendance to an MRI workshop in SpA performed in different cities in Argentina. The number of correct answers was assessed before and after the workshop; good performance was defined as the achievement of 6 correct answers on average between the pre- and post-test. Participants' characteristics were collected. RESULTS: A total of 106 participants were evaluated. Average of correct answers before and after the workshop were 5.3 and 6.8, respectively (P = .0001); 65% of participants achieved good performance. Performance is not associated with the characteristics of trained physicians. CONCLUSION: MRI training workshops in SpA allow rheumatologists to improve recognition of acute inflammatory and structural lesions. The long-term effects of such training need further evaluation.

Introducción: La resonancia magnética (RM) es una herramienta diagnóstica fundamental en la espondiloartritis (EspA) axial. Objetivo: Estimar el porcentaje de aciertos en el reconocimiento de lesiones en RM de EspA y conocimientos teóricos, antes y después de realizar una intervención educativa. Estimar si la buena performance en los test se asocia a características individuales de los médicos. Materiales y métodos: Estudio analítico, con intervención activa, se midieron los resultados antes y después de un test con 10 preguntas antes y después del desarrollo de un taller de imágenes de RM en EspA en ciudades distintas de Argentina. Se consideró buena performance a quienes lograron más de 12 puntos entre las dos evaluaciones. Resultados: Se evaluaron en total 106 médicos. Los resultados de los test (total 10 preguntas) antes y después del taller fueron 53% de respuestas correctas y 68% respectivamente (p: 0.000). El 65% de los médicos alcanzaron una buena performance (más de 12 respuestas correctas). Conclusión: El porcentaje de aciertos fue mayor después del taller en forma general, la buena performance no se asoció a ninguna característica especifica de los médicos evaluados.

Dual-energy X-ray absorptiometry is recognized for measuring bone mineral density. The lack of knowledge can lead to errors both in the acquisition of information and in its analysis and subsequent interpretation. The main errors in Ecuadorian Centers were positioning of the patient to the equipment and incorrect analyzed area. PURPOSE/INTRODUCTION: Dual-energy X-ray absorptiometry (DXA) is recognized as the gold standard for measuring bone mineral density (BMD) with acceptable errors, good precision, and reproducibility. However, the training of operators in different centers and countries is not standardized, and the lack of knowledge can lead to errors both in the acquisition of information and in its analysis and subsequent interpretation. The purpose was to determine the most common errors in the performance of bone densitometry from different imaging centers in Ecuador. METHODS: Cross-sectional descriptive study. We collected DXA scans from different imaging centers in Ecuador. Data from the DXA scan included city of origin, type of specialist that requested it, and densitometry diagnosis. The DXA images provided were analyzed double blind by experts in the field from Argentina. RESULTS: From a total of 141 patients with a mean age of 61 ± 10 years, 93.6% were women. About 78% of the DXA scans came from private imaging centers and 22% from public centers, 95% of all came from the city of Guayaquil. The machines used were Hologic 50.4% and Lunar 49.6%. The densitometric diagnosis was 16.3% normal, 46.1% osteoporosis, and 37.6% osteopenia. A total of 112 left hip and 49 right hip scans were analyzed from which 31.2% and 22.4% had errors in patient positioning, respectively, mainly internal or external rotation. About 140 lumbar scans were analyzed from which 21.4% had patient positioning errors (not centered or not straight). Also in 38.5% the vertebral area did not correspond to L1-L4. About 3.5% had artifacts such as a metal bar or implant. The region of interest was misplaced in 24.1% of the lumbar scans and 19.9% of the femur. CONCLUSIONS: DXA quality standards exist but are often not implemented in clinical practice. When studies are performed incorrectly, it can lead to important errors in diagnosis and therapy. Physicians interested in the management of osteoporosis, although not directly involved in the performance and interpretation of DXA, should be familiar with the protocols to minimize errors and allow the proper use of bone densitometry.

The aim of this study was to evaluate the reliability of the outcome measures in rheumatology (OMERACT) definitions for ultrasound (US) elementary lesions in gout through an image reading exercise. Images from patients with gout (static images and videos) were collected. As an initial step, we carried out a image reading exercise within the experts of the Pan-American League of Associations for Rheumatology (PANLAR) US Study Group (n = 16). The following step consisted in a web-based exercise with the participation of larger number of sonographers (n = 63) from different centers. Images were rated evaluating the presence/absence of any US elementary lesion. Inter- and intra-reader reliabilities were analyzed using kappa coefficients. Participants were stratified according to their level of experience. In the first exercise, inter-reader kappa values were 0.45 for aggregates, 0.57 for tophus, 0.69 for erosions, and 0.90 for double contour (DC). Intra-reader kappa values were 0.86, 0.76, 0.80, and 0.90, respectively. The web-based exercise showed inter-reader kappa values for aggregates, tophus, erosions, and DC of 0.42, 0.49, 0.69, and 0.79, respectively. The intra-reader kappa values were 0.62, 0.69, 0.77, and 0.85, respectively. Reliability was not influenced by the sonographer's level of experience. The reliability of the new OMERACT US definitions for elementary lesions in gout ranged from moderate to excellent, depending on the type of lesion.

La Resonancia Magnética (RM) se ha convertido en una herramienta diagnóstica fundamental en la EsP axial, ya que se considera la mejor técnica para la detección de lesiones agudas a nivel del esqueleto axial. Su utilización ha sido la mayor contribución, en los últimos años, en la comprensión del curso de esta enfermedad. Esto ha conducido a que, en los nuevos criterios de la Assessment of SpondyloArthritis International Society (ASAS) para la clasificación de EsP de predominio axial, constituya uno de los brazos de ingreso a los criterios (brazo de imágenes). Además, la cuantificación de inflamación en RM, se ha convertido en uno de los parámetros de desenlace en los ensayos clínicos


Magnetic Resonance Imaging (MRI) has become a fundamental diagnostic tool in the axial SpA, since it is considered the best technique for the detection of acute lesions at the axial skeletal level. Its use has been the major contribution, in the last years, in the understanding of the course of this disease. This has led, in the new criteria of the Assessment of SpondyloArthritis International Society (ASAS) for the classification of axial SpA, to constitute one of the entry arms to the criteria. In addition, the quantification of inflammation in MRI has become one of the outcome parameters in clinical trials

Rhabdomyolysis has traditionally been a clinical diagnosis with healthcare providers utilizing historical context, physical exam, and laboratory data to arrive at a diagnosis. However, there are myriad imaging findings that may be corroborative and support the presumptive diagnosis. This paper seeks to review imaging findings associated with rhabdomyolysis using different modalities including radiography, sonography, computed tomography (CT), magnetic resonance imaging (MRI), and bone scintigraphy.

Bisphosphonates (BPs) are the most widely used drugs to treat osteoporosis. However, recent reports associated to long-term BPs use with atypical low-impact fractures and prodromal pain. It is estimated that 26% of the cases of atypical fractures associated with the long-term use of BPs show delayed healing or nonunion. Teriparatide [PTH1-34] (TPTD) is an anabolic drug shown to be effective in stimulating bone formation. The aim was to describe the course of a right diaphyseal femoral fracture sustained by a patient on long-term BPs treatment. A 57-year-old postmenopausal Caucasian female presented with delayed healing of a right femoral diaphyseal fracture 10 months after the fracture, despite having received orthopedic treatment. The fracture was preceded by progressive, severe, and bilateral thigh pain. Her medical history included osteopenia that was treated with alendronate over 7 years. On presentation at our clinic, the patient ambulated with the aid of a walking cane. The diagnosis was an atypical right femoral fracture associated with long-term alendronate use. The levels of the following parameters were measured: mineral metabolism laboratory: intact parathormone, 40 ng/mL (reference values (rv): 10-65 ng/mL); 25-hydroxyvitamin D, 40 ng/mL (rv: >30 ng/mL); serum Crosslaps, 318 ng/mL (rv: 80-590 ng/mL); and bone-specific alkaline phosphatase, 76UI/L (rv: 31-95UI/L)]. Magnetic resonance imaging of the left femur was performed, which revealed a diaphyseal stress fracture. She was prescribed 20 µg/day of subcutaneous (s.c.) TPTD (PTH1-34, Forteo; Eli Lilly Co., Indianapolis, IN, United States). A computed tomography scan performed 3 months later showed that the fracture had healed; the patient was able to resume her usual activities. Twenty micrograms per day of s.c. TPD accelerated the healing of the atypical fracture associated with long-term alendronate therapy, allowing a fast recovery of ambulation and quality of life.

Materials such as calcium hydroxide paste and mineral trioxide aggregate are used in apexification treatment of immature permanent teeth, but the search for improved materials with higher characteristics of biocompatibility results in different materials. Biodentine is a tricalcium silicate cement that possesses adequate handling characteristics and acceptable mechanical and bioactivity properties. This report describes the case of a 9-year-old boy who was referred to the Department of Dental Clinic of Querétaro Autonomous University of Mexico. One month prior the patient had suffered a dental trauma of his upper left central incisor and had been treated by another dentist. The clinical diagnosis was previously initiated therapy and symptomatic apical periodontitis. The treatment was apexification with Biodentine. At follow-ups performed at 3, 6, and 18 months after treatment the tooth was asymptomatic. The cone-beam computed tomography scan at 18-month postoperative follow-up revealed continuity of periodontal ligament space, absence of periapical rarefactions, and a thin layer of calcified tissue formed apical to the Biodentine barrier. On the basis of sealing ability and biocompatibility, apexification treatment with Biodentine was applied in the present case report. The favorable clinical and radiographic outcome in this case demonstrated that Biodentine may be an efficient alternative to the conventional apexification materials.

The tree-in-bud pattern is commonly seen at thin-section computed tomography (CT) of the lungs. It consists of small centrilobular nodules of soft-tissue attenuation connected to multiple branching linear structures of similar caliber that originate from a single stalk. Originally reported in cases of endobronchial spread of Mycobacterium tuberculosis, this pattern is now recognized as a CT manifestation of many diverse entities. These entities include peripheral airway diseases such as infection (bacterial, fungal, viral, or parasitic), congenital disorders, idiopathic disorders (obliterative bronchiolitis, panbronchiolitis), aspiration or inhalation of foreign substances, immunologic disorders, and connective tissue disorders and peripheral pulmonary vascular diseases such as neoplastic pulmonary emboli. Knowledge of the many causes of this pattern can be useful in preventing diagnostic errors. In addition, although the causes of this pattern are frequently indistinguishable at radiologic evaluation, the presence of additional radiologic findings, along with the history and clinical presentation, can often be useful in suggesting the appropriate diagnosis.

La dinámica poblacional de Pando muestra una nueva realidad regional, en la que se amalgaman individuos portadores de culturas diferentes. Ellos por su permanencia voluntaria en este espacio sociogeográfico, se van convirtiendo en sujetos de la pandinidad, entendida ésta como una nueva identidad emergente de gran potencialidad en el concierto de la Bolivia multiétnica y pluricultural. Este trabajo mueve a comprender que hoy en día acercarse a otras culturas, a otras cosmovisiones, es imprescindible para una convivencia social pacífica.

We examined the association between geographic distribution, ecological traits, life history, genetic diversity, and risk of extinction in nonhuman primate species from Costa Rica. All of the current nonhuman primate species from Costa Rica are included in the study; spider monkeys (Ateles geoffroyi), howling monkeys (Alouatta palliata), capuchins (Cebus capucinus), and squirrel monkeys (Saimiri oerstedii). Geographic distribution was characterized accessing existing databases. Data on ecology and life history traits were obtained through a literature review. Genetic diversity was characterized using isozyme electrophoresis. Risk of extinction was assessed from the literature. We found that species differed in all these traits. Using these data, we conducted a Pearson correlation between risk of extinction and ecological and life history traits, and genetic variation, for widely distributed species. We found a negative association between risk of extinction and population birth and growth rates; indicating that slower reproducing species had a greater risk of extinction. We found a positive association between genetic variation and risk of extinction; i.e., species showing higher genetic variation had a greater risk of extinction. The relevance of these traits for conservation efforts is discussed.

We study the properties of electrical double layers separated by a charged plate of finite thickness. The nonlinear Poisson-Boltzmann equation is analytically solved for this system. It is shown that, for an unsymmetrically charged, narrow plate, the charged fluids at both sides of the plate are strongly correlated, and the local electroneutrality condition (LEC) is not satisfied. The LEC is satisfied only for an infinitely thick plate. Analytical expressions for the induced charge and mean electrostatic potential are given and analyzed. These findings could be relevant for the understanding of protein adsorption on membranes.

Este caso documenta el espectro imagenológico de una enfermedad poco frecuente y ayuda a identificar un potencial patrón diagnóstico. Se presenta una paciente de 35 años de edad que consulta por dolor dorsolumbar inespecífico, es estudiada mediante Rx simple, Tomografía Computada (TC) y Resonancia Magnética (RM). Los hallazgos radiológicos evidenciados permitieron confirmar la naturaleza benigna de la enfermedad. La linfangiomatosis musculoesquelética es una entidad benigna poco frecuente de etiología desconocida, que compromete a los vasos sanguíneos y linfáticos, produce lesiones quísticas difusas a nivel del esqueleto axial y apendicular y se acompaña de afectación visceral. El empleo de los distintos métodos de imágenes permite realizar el diagnóstico correcto

La linfangiomatosis musculoesquéletica ha sido descripta esporádicamente en la bibliografía radiológica y ortopédica. Este caso documenta el espectro imagenológico de una enfermedad poco frecuente y ayuda a identificar un potencial patrón diagnóstico. RESUMO

La Artrorresonancia Magnética Nuclera (ARM) del hombro es una modalidad diagnóstica que combina las ventajas de la Artrografía convencional y la Resonancia Magnética Nuclear (MN). La inyección intraarticular de Gadolinio distiende y contrasta la articulación, destacándose estructuras intraarticulares que normalmente no se ven adecuadamente en un estudio de RM convencional, entre ellas el lábrum, la cápsula y los ligamentos glenohumerales, el complejo lábrum-biceps y la superficie articular de los tendones del manguito rotador. Las principales indicaciones de la ARM son los estados de inestabilidad glenohumeral, ciertos casos de afectación del manguito rotador y del tendón bicipital y el dolor de hombro de causa no probada con RM previa. El objetivo de este trabajo es el de evaluar la ARM aplicada al estudio de la anatomía y la patología de la articulación del hombro
